Direct Deposit Notification Form

Use this form to notify us of an incoming direct deposit (ACH).

This form must be completed in full by the sender and received by us on the same day that the payment is being sent. If this form is incomplete, or if it is not received by us in a timely manner, funds may be returned to the originating bank.

If you are sending an ACH or direct deposit, please complete and submit this form. This form must be completed in full and received by Forge Trust Co. on the SAME DAY that you are sending the payment. The payment will be credited to the investor’s IRA account one business day following receipt of funds by our bank. If this form is incomplete and/or we do not receive this notification in a timely manner, funds may be returned to the originating bank.

You can submit the form by using one of the following methods.

DocuSign

This is the preferred way to complete the form.  You can fill out the form online and sign it electronically by DocuSign.

DocuSign is a fast and secure way to process your form. It allows you to digitally sign forms without having to go through the hassle of downloading software or mailing the originals.

You will receive an email with a copy of the form after signing is completed.  There is no need to mail in the form.

PDF Download

If you do not submit the form by DocuSign, you can fill out the form manually by downloading the PDF.
