Please consult your tax advisor to determine tax consequences, if any, as a result of taking a distribution. A Form 1099-R will be issued to you for the tax year in which you take a distribution. All 1099-Rs are mailed by January 31st of the following year.
This kit contains instruction, Distribution Request form, Direct Deposit (ACH) Authorization form and Wire Request form.
This form is not required for the following assets: Certificate of Deposit, IRA (“Checkbook”) LLC, Precious Metals, Publicly-Traded Assets, and Real Estate/Raw Land.
