RMD Asset In-Kind Request Form

IMPORTANT: To ensure a transaction is processed by year-end, please submit all required paperwork in good order no later than Dec 1 of the current year.

Use this form to request a RMD Asset In-Kind Distribution from your account.  A Form 1099-R will be issued for all distribution request.

If your RMD requires a liquidation you should make your request by November 15th, however we can not guarantee that the funds will be received in time for us to process your payment by year end. Please check with your investment provider to determine if they will be able to liquidate and deliver funds by December 1st.

You can submit the form by using one of the following methods.

DocuSign

This is the preferred way to complete the form.  You can fill out the form online and sign it electronically by DocuSign.

DocuSign is a fast and secure way to process your form. It allows you to digitally sign forms without having to go through the hassle of downloading software or mailing the originals.

You will receive an email with a copy of the form after signing is completed.  There is no need to mail in the form.

If you do not submit the form by DocuSign, you can fill out the form manually by downloading the PDF.

After you complete the PDF form, you need to deliver the form to us by either email, fax, regular mail or overnight mail.
